||APPLICATION OF PROFESSIONAL NURSING CARE MODEL (PNCM) TEAM AND PRIMARY TEAM IN IMPROVING QUALITY OF SERVICE
IN RSU HAJI SURABAYA
Current nursing care has not reflected the practice of professional services. PNCM is a strategy to improve the quality of professional nursing care in hospitals. The purpose of this study was to analyze the application of the models of PNCM team and primary team in improving the quality of nursing care in Marwah I and Shofa IV wards, RSU Haji Surabaya.
This was a quasi-experimental study. PNCM team was applied in ward Shofa IV, PNCM primary team in Marwah I ward, and control room was in Shofa III ward. Patients sample comprised 50 respondents, while the nurses sample were taken from all nurses on duty in those wards. Independent variables included the application of PNCM team and primary team, and the factors that affected the application of PNCM were nursing staff and nurses' knowledge level, while the dependent variable was the quality of nursing services, namely the level of patient satisfaction and clinical indicators of patient safety. The collection of data on knowledge level and patients' satisfaction was done through questionnaires, and the number of nurses and patient safety data were obtained through observation. Data were analyzed using Wilcoxon Signed Ranks Test and Mann Whitney test, with significance level of α ≤ 0.05
Results revealed that the number of nurses in PNCM Team ward and PNCM Primary Team ward adequate, but not in quality. For the level of knowledge in PNCM primary teams was high, in PNCM team was moderate, and that in control was low. The results of statistical tests of Wilcoxon Signed Ranks Test revealed that the level of knowledge had p = 0.024, showing difference before and after application of PNCM. PNCM application had p = 0.011, satisfaction level had p = 0.012, but patient safety had p = 1.000. Mann Whitney test results for the level of knowledge in treatment wards and control ward had z = - 3.584 and p = 0.000, the level of patient satisfaction z = -2.137 and p = 0.033 for the incidence of decubitus z = -6.272 and p = 0.000, phlebitis rate was z = - 3.219 and p = 0.001 but for the number of patients falls, injuries and wrong drug z = 0.000 and p = 1.000.
From the results of statistical tests, it can be concluded that there was positive relationship between the application of PNCM and the improvement of the quality of nursing care in RSU Haji Surabaya. It is recommended for hospitals to implement PNCM primary team. Nurses are expected to improve knowledge, while other researchers should follow up the study on PNCM.
